Indications/Uses
Type 2 DM in adults when dietary measures, physical exercise & wt loss alone are not sufficient to control blood glucose.
Dosage/Direction for Use
Adult including elderly or w/ mild to moderate renal insufficiency Initially 30-120 mg once daily. Patients at risk of hypoglycemia Initially 30 mg daily. May be given in combination w/ biguanides, α-glucosidase inhibitors or insulin.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to sulfonylureas, sulfonamides. Type 1 diabetes, diabetic precoma & coma, diabetic ketoacidosis. Severe renal or hepatic insufficiency. Concomitant use w/ miconazole. Lactation.
Special Precautions
Hypoglycemia. Monitor blood glucose levels & take exercise regularly. Severe hepatic or renal impairment. Patients w/ G6PD deficiency. Pregnancy (change to insulin before pregnancy).
Adverse Reactions
Hypoglycemia, abdominal pain, nausea, vomiting, dyspepsia, diarrhea, constipation, changes in haematology. Raised hepatic enzyme levels, hepatitis. Cholestatic jaundice. Transient visual disturbances at start of treatment, rash, pruritus, urticaria, angioedema, erythema, maculopapular rashes, bullous reactions eg, SJS, TEN & DRESS. Erythrocytopenia, agranulocytosis, haemolytic anaemia, pancytopenia, allergic vasculitis, hyponatreamia, elevated liver enzymes, liver function impairment (cholestasis, jaundice) & hepatitis.
Drug Interactions
Increased hypoglycemic effect w/ miconazole, phenylbutazone, alcohol, other antidiabetic agents, β-blockers, fluconazole, ACE inhibitors, H2-receptor antagonists, MAOIs, sulfonamides, clarithromycin, NSAIDs. Blood glucose levels may be increased by danazol, chlorpromazine, glucocorticoids & ritodrine, salbutamol, terbutaline. St. John's wort (Hypericum perforatum) prep. Risk of dysglycaemia w/ fluoroquinolones. Potentiation of anticoagulants therapy.
